Cymbalta (IUPAC name - duloxetine) is a drug that needs to be administered very carefully and scientifically, mainly because of the many side effects associated with it. Cymbalta dosage directions as per condition:

Major Depressive Disorder (MDD) - The protocol is to start the Cymbalta dosage at 40mg per day (20mg taken twice) to 60mg per day (30mg taken twice). However, it may also be advisable for some patients to start with a Cymbalta dosage of 30mg once in a day in the first week. Although 40mg per day has shown benefits, it does not mean that 60mg instead can bring better recovery or any additional benefit for the same patient. For continuation it is said that major cases need several months of therapy and Cymbalta should be administered with a dosage of 60mg once daily. It is also important that the patient is regularly under check as dosage amounts may need to be changed accordingly.

Generalized Anxiety Disorder (GAD) - GAD is recognized as a chronic condition. A Cymbalta dosage of 60mg once daily is advised for beginning the schedule for the first week or so. In some case it may be 30mg of Cymbalta once daily to allow the patient time to adjust to the medication. It was earlier believed that 120mg once daily was effective, but it has been concluded by experts that dosages greater than 60mg need not be proportionally beneficiary. Any change of Cymbalta dosage is also advised to be changed with 30mg to avoid difficulties for the patient to adjust. The effectiveness of Cymbalta on GAD patients is not known beyond 10 weeks. As a continuation procedure, the health care provider must be alert to changes and development in the patient through the Cymbalta medication course.

Diabetic Peripheral Neuropathic Pain (DPNP) - The recommended dosage is 60mg Cymbalta once daily. There is no evidence of better effectiveness with a higher dosage. In cases of DPNP, renal complications may be a factor. Mild renal impairment (CLcr >= 30 mL/min) needs no change of dosage whereas patients with renal impairment (CLcr < 30mL/min) are advised to stay off Cymbalta medication. In the continuation stage, DPNP patients must be assessed individually by the health care provider as there is no generic formula for Cymbalta dosage on DPNP patients.

Fibromyalgia - This is also recognized as a chronic condition. Treatment should begin with 30mg Cymbalta once daily for the first week to allow the patient to adjust to the medication. Dosages for individual patients must be decided according to the patient's response thereafter.
